食道がんについて

食道がんとは

The esophagus is a tubular organ that connects the pharynx to the stomach. Its wall comprises several layers arranged from the lumen outward: the mucosa (including the epithelial layer, lamina propria, and muscularis mucosae), submucosa, muscularis propria, and adventitia. Functionally, the esophagus transports ingested food to the stomach through coordinated peristaltic movements and mucus secretion, while also preventing reflux. Esophageal cancer arises from the mucosal lining and may manifest as multifocal lesions, with a higher incidence observed in the mid to lower esophagus. Clinically, cancers confined to the mucosa are classified as early esophageal cancer, those invading only the submucosa as superficial, and those extending deeper as advanced. As the tumor enlarges, it may directly invade adjacent organs, such as the trachea and aorta, and metastasize via lymphatic and vascular routes to regional lymph nodes and distant organs like the lungs and liver.

食道がんの怖さ

Esophageal cancer, when advanced, becomes one of the most excruciating malignancies, necessitating early detection to mitigate its progression. In its terminal stage, not only is the survival rate markedly low and treatment notably challenging, but the physical suffering endured is profound. The esophagus and its neighboring vital structures—such as the vocal cords and airway—are critical to essential functions including swallowing, speaking, and breathing. Initially, involvement of the esophagus impairs the ability to eat, beginning with a persistent throat irritation that escalates to relentless coughing and phlegm production, ultimately compromising sleep and even the intake of water, thereby hindering nutritional sustenance. When the cancer encroaches upon the vocal cords, it results in a loss of voice and severe communication difficulties. In the gravest scenario, the invasion extends to the adjacent airway, leading to significant obstruction, hypoxia, and respiratory failure; while surgical intervention via thoracotomy may be attempted, the dramatically increased risk of pneumonia frequently culminates in fatal outcomes.

飲酒の習慣は食道がんのリスク因子

Acetaldehyde, produced during the metabolism of alcohol, is recognized as a carcinogenic substance. Individuals who exhibit facial flushing after alcohol consumption typically possess a genetically reduced activity of the enzyme responsible for metabolizing acetaldehyde, thereby increasing their risk of developing esophageal cancer. Given that alcohol is inherently toxic, habitual long-term heavy drinking naturally poses significant health risks, including the increased likelihood of esophageal malignancies.

食道がんの自覚症状

Esophageal cancer often presents with subtle, non-specific symptoms that may initially be overlooked. For example, a patient undergoing routine health examinations might first experience vague left-sided chest discomfort, prompting a cardiologic evaluation that yields no abnormalities. Over time, these symptoms may evolve to include chest pain, dysphagia, and unintentional weight loss, with later development of back pain.

食道がんの確定診断

A definitive diagnosis of esophageal cancer is established through biopsy performed during an upper gastrointestinal endoscopy. In this procedure, tissue is obtained from lesions suspected of malignancy and is subsequently examined under a microscope by a pathologist to confirm the presence of cancer. In contrast, while barium swallow studies and computed tomography scans are employed to evaluate disease extent and detect metastases, they lack the early detection capability of endoscopic examination. Furthermore, the utility of tumor markers is limited, as they are typically elevated only in advanced stages and may not consistently reflect the presence of cancer. Thus, endoscopic biopsy remains the gold standard for confirming a diagnosis of esophageal cancer.

Regular comprehensive health examinations (ningen dock) are essential for the early detection and management of esophageal and gastric cancers. Upper gastrointestinal endoscopy, which evaluates both the stomach and esophagus, has demonstrated superior sensitivity in identifying early-stage malignancies compared to radiographic methods such as barium studies. Endoscopic screening enables clinicians to detect subtle mucosal abnormalities and, when necessary, to perform targeted biopsies that facilitate prompt diagnosis and intervention. In contrast, imaging modalities and tumor markers often exhibit lower sensitivity for early disease detection. Consequently, routine health check-ups that incorporate endoscopic evaluation are strongly recommended, as they can lead to earlier treatment, less invasive surgical options, and improved overall patient outcomes.

Early detection of esophageal cancer is critical due to its often asymptomatic nature in the early stages, and current evidence indicates that gastroscopy is markedly more effective than barium studies for identifying early neoplastic changes in the esophageal mucosa. Barium examinations frequently miss subtle lesions, which can result in a diagnosis at an advanced stage characterized by widespread metastases and limited treatment options. In contrast, endoscopic screening permits direct visualization and biopsy of the esophageal lining, thereby facilitating timely diagnosis and intervention. Consequently, routine use of gastroscopy is strongly recommended to improve outcomes by enabling early detection of esophageal cancer.
