局所陰圧閉鎖療法(NPWT)とは

創傷部分を専用の被覆材で覆い、専用の小さな機械で外部から陰圧をかけて浸出液を吸引していきます。浸出液をコントロールすることで細菌の繁殖を抑えながら、陰圧の効果で創傷部の血液量も増やすことができます。肌の環境を整えて創傷を徐々に小さく直していく治療方法です。（局所陰圧閉鎖療法(NPWT)　かまりんヒフクリニック　神奈川県鎌倉市）

物理療法の1つである陰圧閉鎖療法（negative pressure wound therapy：NPWT）は、創面全体を閉鎖性ドレッシング材で覆い、創面を陰圧に保つことにより創部を管理する方法です。創面は専用のスポンジで覆い、125mmHg程度の陰圧で維持します。　ガイドラインでは、「肉芽組織が少ない場合の物理療法」として、「感染・壊死がコントロールされた創には陰圧閉鎖療法を行ってもよい（推奨度C1）」としています。（局所陰圧閉鎖療法は、どんなとき、どのように行う？　ガイドラインに基づく まるわかり褥瘡ケア）

Negative pressure wound therapy (NPWT) applies sub-atmospheric pressure to a wound bed via a suction device attached to a sponge. Closed suction removes excess fluid from the wound, allowing for enhanced circulation, removal of cellular waste, and reduced risk of bacterial contamination. It helps manage extensive wounds that cannot be addressed with primary closure or are too large to be treated with conservative wound strategies.
