アメリカ食品医薬品局（FDA）がアデュカヌマブを迅速承認

FDAは7日「臨床試験の結果、『アミロイドβ』の減少が確認され、患者の症状への効果が合理的に予測される」と評価し治療薬として承認したと発表しました。FDAによりますとアルツハイマー病の新薬が承認されたのは2003年以来18年ぶりで、アミロイドβに作用する治療薬は初めてだということです。（アルツハイマー病の新薬 米FDA承認と発表 エーザイが共同開発 2021年6月8日 17時15分　NHK WEB）

アデュカヌマブ承認までの道のり

外部の専門家から成るＦＤＡの諮問委員会は今回、同薬に対する懐疑的な見方を明確に示した。‥　焦点となったのは、アデュカヌマブに関する２つの試験で相反する結果が出た点。諮問委は、２回目の大規模試験が明らかな失敗となる中、別の試験での成功が同薬の有効性を示す証拠として十分かという点について、否定的な見方を示した。初期の認知症と診断された人の脳のアミロイド斑の水準に対する効果の有無については、５人の委員が肯定的な見解を示した。ただ、６人は「不確か」だとした。諮問委の勧告に拘束力はなく、ＦＤＡが同薬を承認する可能性は依然としてある。（米バイオジェンの認知症薬、ＦＤＡ諮問委「効果証明されず」 2020年11月7日4:00  ロイター）

In unusual move, FDA formed ‘working group’ collaboration with Biogen to conduct further analyses of two aducanumab studies after the Phase III trials were halted. Agency concluded drug’s effectiveness in one of the studies is ‘robust and exceptionally persuasive’ and advised Biogen that it was ‘reasonable’ to submit a marketing application. ALSは遺伝するか

ALSは約1割のケースが遺伝性だとされています。

ALSを発症する年齢

平均発症年齢は60歳前後です。つまりサラリーマンが定年退職するくらいの年齢で発症することが多いということになります。

ALSの罹患率・有病率

ALSの有病率は10万人あたり9.9人で、日本にはALS患者が1万人、世界では40万人存在します。

ちなみに、ある一定期間の間に疾患を発症する割合のことを罹患率、ある特定の時点でその疾患を発症している割合のことを有病率といいます。罹患率は、その病気の原因となる要因が増えたり減ったりしたときに連動して変化しますので、罹患率に注目して病気の原因を探るということができます。

ASLの診断

ALSには客観的なバイマーカーが存在せず、臨床的な診断に依存しています。

ALSの生存期間・予後

ALSは不治で進行性の病気のため現在のところ特効薬が存在しない、やがて死に至る病気です。診断までに1年かかる病気ですが、診断が買う低してからの予後は人によって大きな差がありますが平均３～５年といわれています（気管切開をしない場合）。その差は大きく1年～10年の開きがあるそうです（気管切開をしない場合）。自分で呼吸できなくなるため、気管切開をして呼吸器をつければ、何年間も生き続けることができるそうです（つけないと生きることができない）。

ALSの治療薬

ALSを治す薬は存在しませんん。数か月の延命効果があるクスリとしてリルゾール、進行を抑制する効果があるものとしてエダラボンがあります。残念ながらALSの進行を止めるような薬も、治すような薬も現時点（2024年）では存在しません。

ALSの2種類の病態

Generally, ALS is categorized in one of two ways: Upper motor neuron disease affects nerves in the brain, while lower motor neuron disease affects nerves coming from the spinal cord or brainstem. In both cases, motor neurons are damaged and eventually die. ALS is fatal. The average life expectancy after diagnosis is two to five years, but some patients may live for years or even decades. (The famous physicist Stephen Hawking, for example, lived for more than 50 years after he was diagnosed.) There is no known cure to stop or reverse ALS.
