病理学の教科書 Pathology

病理学の教科書を手に取って読んでみて自分が思ったのは、「これぞ医学！」ということでした。病理学こそ、一番、医学に直結した学びだと言えます。ほとんどの病気は、細胞や組織に何らかの異常を生じているわけですが、病理学の教科書には種々の病気の状態の組織や細胞の写真がたくさん示されて、一体何がどうなっているかを説明しているわけですから。

病理学の教科書と一口にいっても、読者対象に応じていくつかの種類があります。まず、初学者向けに術語の定義から詳しく説明されているもの。それと、アトラスがメインであまり一般的な説明がないもの。あとは、ロビンスなどに代表される大著で、一冊目の教科書には向かないものです。

病理学の勉強方法

初めて病理学を学ぶのであれば、300～400ページ程度の、「通読可能な」教科書を通読＆精読するのが良いと思います。もっと学びたければ、1000ページ程度の本に進むとよいでしょう。

 

初学者向けの病理学の入門書

病理学や医学を初めて学ぶ場合には、まずそこで使われている術語の正しい意味、すなわち定義を理解する必要があります。たとえば、浸潤、肉芽組織、中毒、壊死、細胞傷害といった言葉です。「病理学とは、定義の学問」と病理医の先生が口にしていたのを聞いたことがありますが、「出血」といった日常語にもあるような言葉ですら、しっかりと定義されてから使われます。病理学アトラスの本には、そういった説明はないので、まずは標準的な病理学の入門的な教科書を読むのがよいでしょう。その都度グーグル検索などしていては、いつまでたっても断片的な知識しか頭に入りません。急がばまわれで、まずは、300ページ程度の教科書を一冊通読するのが良いと思われます。

この本が、自分が初めて手にした病理学の教科書でした。図書館で除籍された本を貰ってきたものです。教科書は通読できてこそ教科書という前書きに共感しましたし、実際読んでみて、非常にストーリー性のある説明だと感じました。多くの本が、知識の列挙であることと対照的です。かなり気に入ったのですが、残念ながら図書館で除籍されたくらいですから出版は2006年と古く、その後改訂版は出ていません。病理学の歴史は古いので基本的な記述は問題ありませんが、免疫学など伸展が早い分野に関しては、記述の古さが目立ちます。そのため、人に勧めることはできません。自分は、最新の別の病理学の本と照らしあわせながら、一つ一つ説明が古くなっていないかを確認しながら読むということをやっています。その労力が惜しくないくらいに、良く書かれた本だと思います。

 

カラーで学べる　病理学　第6版　NOUVELLE HIROKAWA 2025年1月20日発行　402ページ

これは図書館の新着図書コーナーで見つけました。「よくわかる専門基礎講座 病理学」と併せて読むために、できるだけ出版年が新しいものをと思っていて見つけたものです。401ページなので、「よくわかる専門基礎講座 病理学」と同じ程度の分量で、読者対象も同じく初学者だと思います。タイトル通り、写真がカラーなのが嬉しい。やはり組織標本はカラーでないと、伝わりません。グラム陽性球菌は青紫に染色され、グラム陰性桿菌は赤～ピンク色に染色されると説明されても、白黒では全くインパクトがありませんが、この本はカラー写真なので非常にわかりやすいと思いました。説明も詳しくて良い本だと思います。第6版まで改訂されていたり、中国語や韓国語の訳本も出版されているというのもうなずけます。

 

標準病理学　医学書院から出ているこの教科書は上記の2冊よりもはるかにページ数が多く（856ページと、倍以上！）、医学部の学生向けです。

新病理学(Qシリーズ)　Qシリーズは自分は発生学を持っていて、なかなかよくできた教科書だと感心していましたが、Qシリーズの他の教科書がどれくらい読みやすいのかは自分は知りません。
